Vendor Invitation and Registration From For Nursing Assistant Appreciation Banquet

Dear Vendor,

The Long-Term Care Ombudsman Advisory Council at East Alabama Regional Planning and Development Commission would like to invite you to take part in our upcoming Nursing Assistant Appreciation Banquet. There are 64 facilities in our 10-county service area, therefore there is a potential to have a large group at this event. The purpose of the banquet is to recognize the essential work done by these valued employees and to encourage them in their everyday responsibilities. As a vendor, we would like your agency/company to display/demonstrate your services and/or provide samples to help pamper these hard-working individuals. The theme is “Caring Hearts Touching Lives” and the color scheme is black and white with accents of red.

For your information, the event details are provided below.  
Date of the Banquet: September 12, 2019
Location: Anniston City Meeting Center, 1702 Noble Street, Anniston, Alabama
Suggested arrival time to set up booth: 2-4 pm
Banquet hours: 5:00 pm-9:00 pm
Number of tables: 1
Number of chairs: 2
Participation fee:  There is no cost for vendor tables, but we are asking that vendors provide a minimum of one (1) door prize (valued at $25).

If you agree to participate, please complete the enclosed Vendor Application Form and return it by August 2, 2019 by faxing it to my attention to 256-237-6763 or emailing to summer.mcneal@earpdc.org. Vendor tables will be limited and will be accepted on a first come, first serve basis.
